Reaffirmation of TIA-only Document (Non-ANSI)

This form is to be used for the reaffirmation of a TIA Standard OR TSB when a vote has been taken at an official standards formulating group meeting approving any of the following actions: reaffirm a TSB OR reaffirm a TIA Standard. This form conveys all the necessary information to the TIA Standards Secretariat so that the document can be processed correctly.

Note: This form can not be completed until the meeting report has been approved by legal counsel and is on file at TIA.
