Ballot Vote Change Form "Blue Card"

The Ballot Vote Change Form is to be used when the voter disapproves the document, the comments submitted by the voter have been discussed by the committee, and a reply letter from the Chair of the committee has been received by the voter. The voter, if they so desire, can then change his/her vote.To enter a ballot vote change, submit the form below electronically or print out and send by postal mail or fax using the contact information at the bottom.
